How to Make a Report

Please be prepared to discuss the following types of questions:

  • Name, address and phone number of person making report.
  • Name, address and phone number of person who was victimized.
  • Accused Person's Name / Description
  • Summary of dates and events and other people who may know about the event you are describing.
  • What parish, city, state did this happen in?
  • Has this been reported to the authorities (Child Protective Services, Police Department, Diocesan authorities)?
  • Does this report involve someone less than 18 years of age?
  • Does this report involve a legally vulnerable adult (an adult who has a legal guardian)?

Where to make a report

You can make a report in any of the following ways:

  • Scheduling an office appointment
  • Scheduling an off site appointment
  • By telephone
  • Through written statement

What happens after a report is made?

Each individual's confidentiality is important to us. We respect your Right to Privacy, Dignity, and Confidentiality.

Whenever you are not sure if a crime has been committed, please call your local law enforcement office or Child Protective Services at 1-888-767-2445 if a child is involved in the report!
