How To Make A Report

Contact Director, Office of Child and Youth Protection

Please be prepared to discuss the following types of questions. 

Name, address and phone number of person making report.
Name, address and phone number of person who was victimized.
Accused Person's Name / Description
Summary of dates and events and other people who may know about the event you are describing.
What parish, city, state did this happen in?
Has this been reported to the authorities (Child Protective Services, Police Department, Diocesan authorities)? 
Does this report involve someone less than 18 years of age? 
Does this report involve a legally vulnerable adult (an adult who has a legal guardian)


Where to make a report

You can make a report in any of the following ways:

Scheduling an office appointment
Scheduling an off site appointment
By telephone
Through written statement

Whenever you are not sure if a crime has been committed,

please call your local law enforcement office or

Child Protective Services at 1 - (888) 767-2445 if a child is involved in the report!
