St. Raphael Roman Catholic Parish Census

 

 

Please read the following very carefully. 

 

Thank you for taking the time to help us update our parish records.

The census form is divided into three separate sections.  First is the Head of Household section.  Every family will

start with this form.  Please complete all fields before submitting. The next section is the spouse form.  This

form is to be completed by the spouse of the head of the household.  The third section is to be filled out for each

additional family member.  Residents of the Household who are 18 years of age or older must register separately.

Be sure to click submit after each section.   Here are a couple of example scenarios.

 

Scenario 1. A husband and wife with one child (Under 18):

    Step1.  Fill out one head of household form. (Click Submit),

    Step 2. Fill out one spouse form (Click Submit)

    Step 3. Fill out one additional family member form. (Click Submit)

 

Scenario 2. A single mother and with three children (one of which is over 18):

    Step 1. Fill out one head of household form for the mother. (Click Submit)

                No Spouse form is necessary for this scenario.

    Step 2. Fill out one additional family member form. (Click Submit)

    Step 3. After you click submit, you will be taken to a confirmation page.  At the bottom of that page

                is an option to return to the census form. Click that Link to go back to the form.

    Step 4. When you go back to the form, it will be blank. (It will not keep your information) Now that

                you are back to the blank form, you can fill out the additional family member form for

                the second child.  Once the form is completed Click Submit.

    Step 5. The third child (18 and over) must fill out a new head of household form separate from

                the parents' head of household form.
